JACE HOME CARE LLC is an Adult Foster Care provider serving adults ages 16 and older who require assistance with daily living. Founded in October 2025 by four nurses, our organization was built on a shared commitment to high-quality, compassionate care. With over 25 years of combined experience in long-term care, home care, and rehabilitation, we understand the importance of both clinical knowledge and human connection.

Located in Blackstone, Massachusetts, we serve individuals and families across Worcester, Essex, and Middlesex counties. At JACE HOME CARE LLC, we care for your loved ones with the same respect, attention, and compassion we would give our own.

We envision a setting in which comprehensive assistance with Activities of Daily Living and Instrumental Activities of Daily Living, along with medication support, meaningful companionship, and emotional encouragement, is delivered with consistency, respect, and professionalism. Through 24-hour supervision and individualized care, we aim to set a standard of safety, stability, and peace of mind for participants and their families while fostering a sense of belonging and well-being.
